Alton Memorial Hospital's Diabetes Management Center can help you manage your diabetes by providing you with the information and resources you need. The American Diabetes Association recognizes the Center for quality self-management education.
Diabetes Management Center Services
The Center offers one-on-one sessions with a diabetes nurse specialist. You will find a variety of services, including:
- Diabetes overview
- Medication instruction
- Insulin pump training
- Diet — carbohydrate counting
- Exercise planning
- Foot care strategy
- Home monitoring of blood sugar and urine ketones
- Sick-day management
- Education on the prevention of complications
- Resources and community services
- Consultation for gestational diabetes (occurring in some pregnant women)
- Consultation for children with Type 2 diabetes
A physician referral is required to receive Center services. Medical plans often cover diabetes education. Consult your provider.
Diabetes Support Group
If you or a loved one has diabetes, the Diabetes Management Center hosts a support group from 12:30-1:30 p.m. on the last Tuesday of each month.
To visit the Diabetes Management Center, please park outside the hospital's Olin Wing (orange awning). Go in that door and the Diabetes Management Center is the first office on the left (Room G-267).
